The Adult Foster Care is a MassHealth funded program designed to provide individual home care services to adult clients with medical diagnosis who require assistance and/or supervision for their Daily Living Activities to maintain independent lifestyle in a home setting.
Bay State AFC LLC provides nursing services by a Registered Nurse and care management by a Social Worker with a periodic home visit to train, provide support, health education topics, and monitor clients’ health status and adjustment to the Adult Foster Care program. The care giver will receive training, support and supervision to assure the client’s well being.
The Participant will receive personal care services in a home-like setting that allows the participant to enjoy a private home atmosphere.
Bay State AFC LLC will provide 24-hour emergency support
(Nursing and Case Management staff)
